Orange County Surf Coaching Liability Under 18

I give my son/daughter permission to participate in surf coaching and training with (Orange County Surf Coaching) without myself being present. I understand that although I am not personally present, I am responsible for my child.

This agreement releases (Orange County Surf Coaching, Their employees, Independent contractors, and volunteers) from all liability relating to injuries that may occur (surfing, training, or being transported by a vehicle)

By signing this agreement I agree to hold (Orange County Surf Coaching) entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.

I also acknowledge the risks involved in (Surfing). These include but are not limited to ( Injury, drowning, or death) I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

By signing below I forfeit all rights to bring a suit against (Orange County Surf Coaching) for any reason. In return, I will receive (surf coaching) I will also make every effort for obey safety precautions as listed in writing as explained to me verbally. I will ask for clarification when needed. I am also agreeing to letting orange county surf coaching to use video and photos of myself or my child for marketing purposes.
